Your Appointment

All patients 
If you are new to our kidney treatment practice, we want to make your appointment as pleasant and easy to manage as possible. Appointments should be made with our staff during regular clinic hours. We will do our best to accommodate your schedule with the soonest possible appointment. We kindly ask that you give us 24 hours notice if you are unable to keep your appointment. We will attempt to contact you by telephone prior to you appointment.
What to bring
When you arrive for your office visits, you will be required to provide the following items:
  • Completed patient enrollment forms (PDF)
  • All insurance cards
  • Driver's license or photo I.D.
  • Medications (actual bottles or a list)
  • Medical records, X-rays, and other pertinent laboratory tests and results (if available)
  • Insurance co-payment
KSOSN have 11 locations throughout Las Vegas and the southern Nevada area. Our nephrologists and kidney specialists team work with you and your primary care physician to help you understand kidney disease.
